We want you and your family to be involved in the health care decision-making process. By working together with you, we will provide you with the highest quality of care. One of the ways we do this is through medical homes.
What is a Medical Home?
A medical home is a way of offering health care services that empowers patients and families to be more involved in care. It replaces the traditional way primary care is organized and delivered. You have an ongoing relationship with a trusted provider and a team of health care professionals of your choice who are actively involved in your health. Your provider and care team partners with you and your family so you can make well-informed health care decisions together.
A medical home delivers primary care that is:
- Committed to safety and quality: Your provider and care team are committed to the safety and quality of your care while working with you and your family to make health care decisions.
- Patient- and family-centered: You and your family work with your provider and care team to make health care decisions that respect your wants, needs and preferences. You also receive education and support you need to make decisions and participate in your own care.
- Comprehensive: Your provider and care team work together to ensure you receive recommended preventive care, acute care and chronic disease management.
- Coordinated: Your care is organized across the broader health care system and includes specialty care, hospitals, home health care, and community services and support.
- Accessible: You can access services with shorter waiting times, “after-hours” care and 24/7 electronic or telephone access.
As partners, we also need your involvement in your care by:
- Providing a full medical history
- Supplying a list of current medications
- Giving information about visits to specialists
- Giving a complete health status report
- Asking questions about recent test results
- Providing information about how you care for yourself
- Giving your health care providers data (or where to obtain it) from recent hospitalizations, specialty care, or emergency department visits

In a medical home, your care team is available when you need them. You can obtain care and clinical advice during office hours by calling your primary care provider. Staff will help you get an appointment quickly, even on the same day when needed.
Should you need care after our office has closed, you may call the same phone number. The answering service will connect you to a nurse, your provider or the provider on-call.
You are also able to communicate with your provider and your care team using secure electronic messaging in the myCepEsperu patient portal. Your provider can give you more information and instructions to set up an account.
